Janice Garcia, 28, was charged with driving under the influence/serious bodily injury, Miami-Dade Jail records showed.
According to the Miami-Dade Sheriff’s Office, on April 11, Garcia was heading northbound on Southwest 99th Court near 85th Street.
An arrest report said deputies were working on another case in the area when one of them saw headlights approaching.
One of the deputies said the vehicle was flashing its high beams at him, and the other deputy said the vehicle was close to the edge of the roadway, and he tried to move out of the way, but was struck, the report said.
The deputy, the report said, was propelled through the air and suffered life-threatening injuries and was taken to the hospital.

After the deputy was struck, another deputy arrived at the scene and spoke with Garcia, who was distraught, had slurred speech, and bloodshot watery eyes, the report said. The deputy also detected the smell of an alcoholic beverage on her breath.
Garcia then agreed to perform field sobriety tests and have her blood drawn for testing, the report said.
Weeks after he blood was tested, the report said, her BAC concentration was measured at 0.156g/100ml and 0.154g/100ml.
The legal limit in Florida is 0.08 grams per 100 milliliters.
Garcia surrendered herself on Sunday.
During her court appearance on Monday, a judge set Garcia’s bond at $15,000 and ordered her to be on house arrest.
The deputy who was injured after being struck received a skin graft on April 11 and had angle surgery on April 29.
