A Hialeah assisted living facility was ordered to stop operating Friday after state health regulators found multiple violations.

The Florida Agency for Healthcare Administration suspended Our Dream ALF’s license, meaning the owner, Daisy Fundora and associates, can no longer operate.
The state health regulators issued a 34-page emergency suspension order, which was taped to the front door – listing violation after violation, from not giving residents their medications, to inadequate staffing.
The order noted how earlier this month, inspectors found that the facility “only (had) two staff… providing care for over 29 residents, many of whom have mental health conditions and other needs.”
On Sunday, July 26, state health regulators say a resident was found unresponsive and died at the hospital.
According to the suspension order, “staff members failed to perform CPR appropriately, failed to use the automated external defibrillator (AED), and failed to contact 911 for emergency medical services in a timely manner for the resident who passed away.”

Earlier Friday, a woman showed up saying she couldn’t get a hold of anyone and that she was looking for two patients.
NBC6 reached out to State Rep. Alex Rizo, who represents the area where the facility is located.
“Our seniors deserve to be treated with dignity, compassion, and the highest standard of care,” he said in a statement. “When an assisted living facility fails to meet those obligations, especially when the health and safety of vulnerable residents are at risk, swift action is essential. I appreciate AHCA’s decisive response and will continue working to ensure every senior in our community is protected and receives the quality care they deserve.”
NBC6 reached out to state health regulators for more information on the patients, but they didn’t want to comment beyond what’s in the suspension order. NBC6 also called a number listed for the owner, but the person who answered declined to comment.
